Gablofen® (baclofen injection) is an AP-rated generic drug. As an AP-rated generic equivalent, Gablofen can be substituted for Lioresal® Intrathecal (baclofen injection).
Gablofen is available in the same standard concentrations as Lioresal® Intrathecal (baclofen injection) (50 mcg/mL, 500 mcg/mL, and 2,000 mcg/mL).
Intrathecal baclofen (ITB) centers should use the following pre-existing intrathecal baclofen codes with Gablofen:
- J0475 Injection, baclofen, 10mg
- J0476 Injection, baclofen, 50mcg for intrathecal trial (screening)
Gablofen's reimbursement coverage includes Medicare Part B, Medicaid, and the Medicare Part D prescription drug plan.
